I am currently dealing with this and dementia - so I have my plate full

Once my father told me he was on the phone when his cousin passed on - he had been talking to their wife when things took a turn for the worse and they forgot to hang up the phone - I think it was the death rattle of the dying person that shook him to the core - it took a while before the person passed on and my father somehow could not bring himself to get off the phone

I didn't know a lot of things you mentioned in this post - it was a difficult yet eye opening read
